As a Full-time employee, you will:

Requirements

As the Outreach Worker, you will be responsible for supporting women and children in the SFL Transitional Housing Program who are seeking to move forward from the experience of relationship/family violence. In this role you will promote self-empowerment, wellbeing and goals through individualized support plans, services, and daily programming. In addition, you will also be responsible for maintaining the daily operations of Dol-Mar Manor.


As an Outreach Worker, you will:


  • Develop and maintain an active residential and outreach caseload; process referrals, identify and update resources/referral sources.

  • Support increased safety, wellbeing, and empowerment through case management services, encourage women to identify their own strengths, needs and goals, create support plans/goals, connect to community resources, and advocacy.

  • Maintain a continuum of support through 1-1/ group sessions and informal engagement.

  • Maintain timely, accurate, factual, and professional documents in accordance with SFL policies and procedures.

  • Works collaboratively with referral partners to promote seamless services to clients and enhance partnerships with referral organizations

  • Facilitates weekly life skills programming and develops/implements client-wellness activities.

  • Maintains a strong communication linkage with other SFL service areas, identifies opportunities for collaboration and client support.

  • Contributes daily to maintain a high standard of health, cleanliness, and safety in all areas of the facility. Complies with all emergency processes and risk management.
